The Family Medical Leave Act (FMLA-Form)




         On June 14, 2010, I contacted to the Target’s third party Administrator that manages the FMLA, to request an Unpaid Medical Leave Absence. The administrator provided to me the FMLA Form as well the Team Member’s Own Serious Health Condition information. On June 15, 2010, paradoxically, the Administrator denied the request for a paid medical leave of absence with short-term disability (STD) I never requested my absence to be paid because I did not have insurance at that time and never requested for a paid leave absence. The Administrator conceded 15 days to submit the FMLA Form. Due to my high-risk pregnancy condition, my medical records were sent to a special Health Care Center. It’s caused delays in the approval of the FMLA form. 
        On July 22, 2010, The FMLA Form was completed and signed by the health care provider representative who confirmed “4.The team member needs to miss work: Continuously-An uninterrupted absence for a single illness or injury” …. “5. Start Date: Date team member is/was first unable to work due to the serious health condition(s) above 6/14/10”… “6. Return Date: Date team member can return to work at his or her normal schedule: 10/20/2010”.   In this date, On July 22, 2010, following the precisely instructions from Benefits Administrator to submit the FMLA form, I proceed to send using USPS the FMLA approved.
